This is to confirm that Name of Company/Employer  is offering you employment for the Semester (Fall, Spring or Summer) Year (2008)  assignment as Job Title with             Start Date: May 15, 2008 and End Date: August 20, 2008.

This employment will serve as Curricular Practical Training (CPT) associated with your academic program at The University of Texas at Dallas. The location of your training will be Job Location Address, Job Location City,  State and Zip.

Your Co-op Manager/Supervisor will be Name of Supervisor, Title and Department.  The Supervisor’s telephone number  and e-mail are: telephone number and email.

You will be expected to work  XX hours per week and will receive $XX.XX per hour.  (CPT's may be part-time or full-time; part-time CPT authorization limits student to a maximum of 20 work hours per week. Any work over 20 hours requires a full-time CPT.  If a student begins a part-time CPT and there is agreement between the employer and the student, a new CPT for full-time employment may  be obtained to replace the part-time CPT. A new offer letter should be submitted to the Industrial Practice Programs office for the authorization process and to obtain a new I-20 before starting the full-time schedule.)
